Supporting Documentation/Medical Evidence

For the purposes of a Student Support Plan, you will need to provide one of the following:

  • Medical evidence in the form of a diagnostic report, a letter of confirmation from a GP or consultant, evidence listed on a hospital letter or confirmation from any other relevant medical professional.
  • Detailed educational documentation such as an EHCP report.

 

For the purposes of a Disabled Students Allowance (DSA) application, your funding body will need you to provide:

  • Medical evidence from a GP, consultant or a diagnostic report. This should include information on how the disability/condition impacts your day-to-day life and that it is long term (longer than 12 months).
  • Alternatively, there is a DSA Disability Evidence Form which can be completed and signed by a doctor to confirm the condition.
